Online Survey Service

Faculty, staff, and students now have access to a service that allows them to create and administer online web-based surveys for research and other purposes.

To create and administer an online survey please complete the following steps:

1. Review the Online Survey Service features.
   
2. Review the Online Survey Service limitations.
   
3. Read the Terms of Use, which explains the official policies and procedures which you must follow when using the Online Survey Service.
   
4. Request an account by completing the Online Survey Service Request Form.
